Superior Court Downloadable Court Forms and Instructions Last Updated: August 30, 2007 About Self-Service Center's Forms We have many court forms to help people help themselves in court. You can download forms (with the exception of domestic violence paperwork) from this site 7 days a week, 24 hours a day. All downloading is free.

Forms are assembled in packets, with all the forms you need for a particular process all grouped together, so you do not leave out anything. If you know of a specific document you need feel free to visit our Alphabetical List of Forms.

Also, the forms have instructions so you will know how many copies you need, where to take the forms, and what are the next steps.

We do not help you fill out the forms, but our forms are easy for you to do, or you can ask a friend or neighbor to help you.

Downloading or Picking Up Forms Below are links to downloadable forms listed by category (Civil Cases, Family Court, etc.) We update packets regularly, so if we do not have what you need now, check back later.

You can also come in person to pick up the forms at the Self-Service Centers in both the Phoenix and Mesa Superior Court locations.

Filing of Papers Filing of papers is to be done at either Superior Court location. There is usually a fee to file papers with the court, but it can be deferred or waived if you qualify. Click here for further information regarding the filing of papers.

Ordering Forms by Mail If you would prefer to receive your forms by mail, click on the Mail Order Form link below. You will be able to print the order form out. Choose which forms or packets you want, include the correct amount of money for the packets PLUS postage fee and return to the Self-Service Center. Your forms will be mailed to you via USPS. If you do not include funds for the packets and postage, your forms will not be sent.
