Self-Service Center

The Self-Service Center is a program provided by the Maricopa County Superior Court Law Library, to help individuals help themselves in Court. The Self-Service Centers and website provide legal information, forms, services and procedural information for self-represented litigants in order to increase understanding, access to the justice system, and more effective participation in court processes.

About Forms

The Self-Service Center has thousands of legal forms available in English and Spanish. All the forms you need for a particular process are grouped in packets and contain detail instructions that explain how many copies you need to make, where to take the forms, and the necessary steps. Individual forms are also available if you are looking for a specific form rather than an entire packet of forms and instructions. All forms are in fillable format. Some family court forms are also available through ezCourtForms, a quick and easy user-friendly interactive computer software program.

Getting Forms and Instructions

There are several different ways to get the forms you need. You can download forms from this site 7 days a week, 24 hours a day. Downloading is free but there is a printing fee if you use a public computer. Forms can also be purchased from the Self-Service Center at the Phoenix, Northeast, Northwest and Mesa Superior Court Locations. Some family court forms are also available through ezCourtForms, a quick and easy user-friendly interactive computer software program.

Please note: Domestic violence forms are not available online. You must come to one of the Self-Service Center locations for this paperwork.

Legal Advice and Help with Forms

Court staff cannot give legal advice or help you fill out forms. If you need legal advice or help filling out forms, you have several options:

  • You can see if the Court offers a workshop on the specific form you need to fill out.
  • You can click on the Self-Help and Educational Videos section below to see if there is an educational video related to the topic for which you need assistance.
  • You can contact the Law Library for other low-cost attorney referral resources and for other legal resource information.
  • You can contact an attorney from our Court's Attorney Roster.

Filing Forms

Your completed papers must be filed at one of the Superior Court locations. There is usually a fee to file papers with the court, but it can be deferred or waived if you qualify. Before filing documents with the Court, consult with an attorney to help guard against undesired and unexpected consequences. Please see the Clerk of Court website for more information about filing your papers.
