Superior Court DISCHARGE/RELEASE OF FUNDS

ATTENTION: Use the forms on this page if the following factors apply to your situation:

You have been appointed guardian and/or conservator for a minor, AND
The minor reached age 18 or there is another good reason to stop the guardianship and conservatorship, AND
You want to be discharged as guardian and/or conservator, AND/OR
You want a court order to release restricted funds to the minor.
