Superior Court TO PREPARE FOR HEARING ON GUARDIANSHIP/CONSERVATORSHIP FOR AN ADULT

ATTENTION: Use the forms on this page if the following factors apply to your situation:

You want to have the court appoint a guardian and/or conservator for an adultor a person at least 17.5 years of age who will need a guardian and conservator as an adult, AND
You gave or will give notice to all interested parties, or will do so, AND
A court hearing scheduled, AND
The person to serve as guardian and conservator completed or will complete any court required training before the hearing,AND
You need the paperwork to prepare for and take to the court hearing.
