Superior Court PREPARATION AND COURT HEARING APPOINTMENT OF GUARDIAN FOR AN ADULT

ATTENTION: Use the forms on this page if the following factors apply to your situation:

You want to have the court appoint a guardian for an adult or a person at least 17.5 years of age who will need a guardian as an adult, AND
You have or will give notice to all interested parties, AND
You have a court hearing scheduled, AND
The person to serve as guardian completed or will complete any court-ordered training BEFORE the hearing, AND
You need the paperwork to prepare for and take to the court hearing.
