You have a court order for child support or spousal support BUT you do not already have an "Income Withholding Order", AND / OR
You are the person entitled to receive support; there is past due support owing, and you want to receive monthly payments on that past due amount, OR
You are the person who has been ordered to make the payments and you want to voluntarily request an "Income Withholding Order".
