SSC: To Get a Consent Decree for Paternity

Superior Court TO GET A CONSENT DECREE FOR PATERNITY

ATTENTION: Use the forms on this page if the following factors apply to your situation:

You or the other party filed a Petition for Paternity, AND,
You and the other party agree to all the terms of the paternity such as who is the father and mother of the child(ren), child Legal Decision Making (Custody), parenting time, and child support, AND,
You and the other party will sign the papers to have the paternity judgment/order by consent (agreement), AND,
You and the other party have attended or will attend the Parent Information Program class before you file a Consent Paternity Judgment/Order.
