ATTENTION: Use the forms on this page only if the following factors apply to your situation:

• You have filed a petition for one of the following:
• Divorce with minor children,
• Legal Separation with minor children,
• Annulment with minor children,
• Paternity,
• A first court order for custody, support and/or parenting time AND
• You served the other party with the court papers AND,
• You have either filed the Acceptance of Service signed by the other party OR the affidavit proving the type and date of service has been filed with the Clerk of the Court AND,
• The other party has not filed a written response within the time frame set by law (check the court file to be sure this is true) AND,
• You want to proceed with the court case by default and get a default court order.

This packet contains court forms and instructions to apply for a default hearing if the other party did not respond. Be sure the documents are in the following order:
