ATTENTION: Use the forms on this page if the following factors apply to your situation:

You are a resident of the county where you are filing this request, AND,
You want to ask the Court to change your name, AND,
You are an adult 18 years or older, AND,
You are the parent of at least one minor child.

DO NOT USE THESE FORMS TO CHANGE YOUR NAME BECAUSE YOU GOT MARRIED.

NOTE: You do not need to request a name change through the Court if you get married and want to use your spouse's last name.
