We have many legal forms available in English and Spanish to help people help themselves in court. All the forms you need for a particular process are grouped in packets. Also, the forms have instructions that explain how many copies you need, where to take the forms, and what the necessary steps are.

If you need help filling out forms, you have several options. You can contact an attorney from our Court’s Attorney Roster, you can see if the Court offers a workshop on the specific form you need to fill out, or you can contact the Law Library for other low-cost attorney referral resources. Court staff cannot give legal advice or help you fill out forms.

Getting Forms

There are several different ways to get the forms you need. You may download forms from this site, purchase forms from a Court location, use ezCourtForms or purchase forms by mail order. Below are links to downloadable forms listed by category (Civil Cases, Family Court, etc.) You can download forms from this site 7 days a week, 24 hours a day. Downloading is free unless you are at a public computer with charges for printing. If you want to pick up the forms in person, you can do this at the Self-Service Centers at the Phoenix, Northeast, Northwest and Mesa Superior Court Locations. Also, if you are looking for domestic violence forms, they are not available online. You must come to one of the Self-Service Center locations for this paperwork. If you would prefer to receive your forms by mail, click on the Mail Order Form link below. You will be able to print the order form out. Choose which forms or packets you want, include the correct amount of money for the packets PLUS postage fee and return to the Self-Service Center. Your forms will be mailed to you via USPS. If you do not include funds for the packets and postage, your forms will not be sent.

Filing of Papers Your completed papers must be filed at one of the Superior Court locations. There is usually a fee to file papers with the court, but it can be deferred or waived if you qualify. Before filing documents with the Court, consult with an attorney to help guard against undesired and unexpected consequences. Please see the Clerk of Court website for more information about filing your papers.
